This position is for a Home Health Physical Therapist responsible for providing physical therapy services to patients in their homes within Seaside, OR, and surrounding areas. The role involves patient care, documentation, and collaboration with the healthcare team, with an emphasis on efficient scheduling and minimizing travel time.

Responsibilities:
- Provide physical therapy services to patients in their homes.
- Conduct patient assessments and develop treatment plans.
- Document patient progress and maintain accurate records using EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/Oasis.
- Collaborate with the healthcare team to ensure coordinated patient care.
- Manage a caseload of patients, potentially visiting up to 6 patients per day, aiming for 30 productivity points.
- Maintain own transportation for home visits.
Required Skills & Certifications:
- 1 year of skilled home health experience.
- BCLS certification.
- Active Physical Therapist license.
- Must have own transportation and car insurance.
- COVID vaccination primary series and one booster.

Special Considerations:
- Traveler is expected to have their own transportation.
- Facility can call off UP TO one shift per pay period (every two weeks).
- Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.
- Business casual attire, no jeans, scrubs allowed but not required (no specific color).
Scheduling:
- 8-hour day shifts.
- No 4x10s or 3x12s shifts are accommodated.
