Department Details Join our innovative Hospital at Home team! This nurse would work in a dual role providing both in-home cares and virtual nursing care. Nurses would assist in screening all admitted patients for eligibility for Hospital at Home program, in which the patient would receive 2 in-home nurse visits to complete medication passes, assessments and other nursing cares needed. In between the 2 in-home visits, the nurse utilizes technology to care for patients virtually, and assisting in provider virtual visits.
The schedule would be days/nights rotating with every other weekend and every other holiday.

Responsibilities * Plans and coordinates patient care, assessment, education, triage and various other nursing interventions
* Collaborates with colleagues, including physicians, to plan, implement and evaluate care
* Empathetically cares for patients during all stages of preventive services, health maintenance, diagnosis, treatment and follow-up along the continuum of care
* Demonstrates competency and uses comprehensive nursing expertise
Qualifications Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor’s Degree in nursing preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.
